Path Assist Info Booth

Representatives from the Path Assist program will be tabling @ HWS On Friday March 3 and March 31 to answer questions and register folks for services.

The goal of Path Assist is to help individuals address issues like housing, food, transportation, access to health care and more before they become serious problems. Path Assist is a team of local Community Health Navigators to help clients improve their quality of life and get better access to the services they need. Clients will be assigned to a CHN who will listen to their needs, help create goals, and connect them to local resources and services that might be helpful. Additionally, clients who screen positive for a transportation challenge and participate in the program are eligible for a $25 gas card or bus pass.
